​New Patient
We regularly update the list of our doctors accepting new patients.
*If you are applying for a doctor not listed above, do not send your intake application without prior pre-approval. It will not be processed and will be rejected.
**Some of our doctors may also have some capacity to take on family members of existing patients. Please contact our clinic via phone for more information before sending any emails or intake inquiries.
***Our clinic does not offer internal patient transfers. Once you have an assigned doctor or nurse practitioner at Nava Medical Centre, we do not transfer you to another doctor in the same clinic. If your family doctor or nurse practitioner is retiring or leaving Nava Medical Centre, we do not transfer or re-assign you to another Nava Medical Centre doctor as all doctors are at full capacity. If you would like your files to be transferred to another clinic, please call our office for more information.

Only follow these steps to register if we have doctors accepting patients.

Instructions for New Patients

To become a new patient at our clinic, please follow the steps below to complete and submit your intake forms. This process is essential for us to provide you with the best care possible.
STEP 1: Obtain the Necessary Intake Forms
Please download the required forms below:

1. Form A (Policy Form): Mandatory for all new patients, regardless of age. Include your designated doctor’s name on the first page.​
2. Form B (Adult History Form): Required for patients aged 18 or older.​
3. Form C (Pediatric History Form): Required, to be completed by parents or legal guardians for patients under 18 years old.​
4. Transfer of Records Form: Required if you have a previous family doctor or have been a patient at another clinic. If not applicable, you may skip this form.
STEP 2: Fill Out the Forms
​Carefully fill out all applicable forms. It is crucial for adults to complete both the Clinic Policy Form and the relevant History Form.
STEP 3: Submit the Completed Forms
Once completed, email your forms admin@navamedicalcentre.ca with the subject line “New Patient Request – [Name of Doctor]”. For example, “New Patient Request – Doctor Last Name.”Include a Copy of Your Care Card: For identification purposes, please attach both the front and back copies of your care card. If you are uncomfortable sending this information via email, you may bring a hard copy to the clinic later.
What Happens Next?
After receiving your completed forms, we will begin processing your application. This process may take a few weeks to a few months, depending on doctor’s availability. We will contact you with further instructions or to schedule your first appointment.
*If we are not accepting patients, please not do not submit your intake application as it will not be processed.
**Please be aware of our zero-tolerance policy which is available on our front page as we do not tolerate any forms of abuse or harassment.
***Please note: Incomplete intake forms will not be processed so please follow the instructions carefully.
