Instructions for
New Patients

​
​

To become a new patient at our clinic, please follow these steps to complete and submit your intake forms. This process is essential for us to provide you with the best care possible.
​New Patient Intake Process

We regularly update the list of our doctors and nurse practitioners who are accepting new patients

*Our clinic does not offer internal patient transfers. Once you have an assigned doctor or nurse practitioner at Nava Medical Centre, we do not transfer you to another doctor in the same clinic. If your family doctor or nurse practitioner is retiring, leaving or for any reason will not be available in Nava Medical Centre, we do not transfer or re-assign you to another doctor at Nava if all of our doctors are at full capacity. If you would like your files to be transferred to another clinic, please call our office for more information.

** You can send us your filled intake forms to be in our wait list, even if none of our providers are accepting new patients.
STEP 1: Obtain the Necessary Intake Forms
Please download the required forms below:

1. Form A (Adult History Form): Required for patients aged 18 or older.​

​​​​​​
​
2. Form B (Pediatric History Form): Required for patients under 18 years old, to be completed by parents or legal guardians.​

STEP 2: Fill Out the Forms
​Carefully fill out the applicable form.
STEP 3: Submit the Completed Forms
Once completed, email your forms to admin@navamedicalcentre.ca with the subject line “New Patient Request – [Name of Specific Provider or Any Available Provider ]”. For example, “New Patient Request – Doctor's or Nurse Practitioner's Last Name.”
Include a Copy of Your Care Card: For identification purposes, please attach both the front and back copies of your care card. If you are uncomfortable sending this information via email, you may bring a hard copy to the clinic later.
What Happens Next?
After receiving your completed forms, we will begin processing your application. This process may take a few weeks to a few months, depending on doctor’s availability for accepting new patients, and our wait list. We will contact you with further instructions or to schedule your first appointment.

**Please be aware of our zero-tolerance policy which is available on our front page as we do not tolerate any forms of abuse or harassment.
***Please note: Incomplete intake forms will not be processed so please follow the instructions carefully.

Appointment options

You can book your visit In clinic vs phone appointments

If you book an in-clinic visit, you can easily switch it to a phone appointment at any time. Just let us know (by phone or email).

However, if you book a phone appointment and later want to switch to an in-person visit, this cannot be done always on the same day. Our clinic may be fully booked, so we can't guarantee immediate availability for in-person visits on the same day.

If you have a phone appointment and feel you need an in-person visit, please ask your doctor during the call. They can arrange an in-person appointment if necessary.
